About the Role Bridge Street Surgery in Downham Market has a fantastic opportunity for an experienced practice nurse to join our team on a part-time basis, with the hours/days to be discussed at interview. The main responsibilities of the role include:

Managing chronic disease including the diagnosis, monitoring and development of individual management plans.

Experience of diabetes and respiratory disease.

Checking compliance with and adherence to appropriate treatments using an holistic patient-centred approach.

Identifying the impact of treatment and implementing or recommending changes as appropriate.

Assessing routine wounds including trauma.

Undertaking patient health checks.

Recognising issues and gathering sufficient information to refer ( drugs; domestic violence; child protection; vulnerable adults; senior patients; social problems).
