**PURPOSE:**
The Population Health Strategy Specialist Practice Consultant is responsible for driving measurable quality, utilization, and cost improvements for physician practices and health systems participating in CareFirst commercial and government valuebased care programs with specialist providers. This role partners directly with assigned provider organizations to analyze population health and performance data, identify gaps in quality and efficiency, and implement practice transformation initiatives aligned with program and contract metrics.

The Specialist Practice Consultant serves as an internal subject matter expert on specialist strategy and a trusted advisor to physicians, practice leadership, and health system executives, translating data insights into actionable strategies related to care coordination, risk adjustment, quality performance, access and total cost of care management.
