Our Medicare Population Health Programs team is a centralized resource that supports care team and practice operations within Amazon One Medical Seniors. As a member of the Medicare Population Health Programs Team, the Population Health Coordinator will work to support our quality strategy by targeting outreach to patients to close gaps, manage data from multiple payers as it relates to quality, implement medication adherence standard work and/or support operations to improve the health outcomes for our patients.
