About the Role:

As a Placement & Brokerage Officer, you’ll play a vital part in our Care Arrangement Team, coordinating timely and effective care packages for adults with assessed needs. You will be instrumental in supporting safe hospital discharges, brokering care that enables people to return home with the right support, and working closely with care providers to match individuals to services that meet their needs.
