**What You’ll Do as a Home Health Physical Therapist:**



+ Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician.

+ Evaluate patient needs and develop rehabilitation goals to improve mobility, strength, balanc...
