Physical Therapist - Home Health - Full Time - York/Fillmore/Saline Counties, NE

Eventide's Home Health Care in York/Fillmore/Saline Counties, NE is seeking a Physical Therapist to work full time 8a- 4:30p. The Physical Therapist is responsible for providing Physical Therapy services to Eventide's Home Health Care Seniors/residents/clients with physical or cognitive disabilities to maximize functional independence.

Essential Functions

• Evaluates clients utilizing appropriate diagnostic muscle, nerve, joint and functional ability tests.
• Performs Physical Therapy treatment procedures or delegates to a licensed Physical Therapist Assistant based on initial and on-going client evaluation.
• Develops and implements the client's plan of care in collaboration with the physician, other consulting health care professionals, family/caregiver according to agency procedures and state and federal regulations.
