Position Overview
RESPONSIBILITIES
I. JOB SUMMARY:
• Conducts interviews with patient and/or caregivers, contacts outpatient providers/pharmacies, and utilizes other resources to obtain the most complete and accurate list of all medications that the patient is taking (including name, dosage, frequency route, and last dose) prior to admissions to the hospital.
• Enters medication list into the inpatient electronic medical record.
• Escalates any discrepancies that need further clarification to the covering clinical pharmacist.
• Performs duties of a Pharmacy Technician as needed.

A. ESSENTIAL FUNCTIONS:
