Position Overview
Position Summary:
The Population Health Pharmacist is responsible for improving the health of populations through optimized medication management, evidenced based prevention strategies, and promoting cost effective care. The Population Health Pharmacist is embedded within a multidisciplinary clinical team, works under a collaborative practice agreement with the physician(s), and provides telephonic and/or virtual interventions.
Education, License & Cert:
Graduate of an ACPE-accredited School of Pharmacy with a Doctor of Pharmacy (Pharm.D.) degree. Current Licensure as a pharmacist in the state of New York and in the state of Pennsylvania. Completion of a pharmacy residency program (PGY1) or equivalent clinical experience. Board certification in ambulatory care pharmacy (BCACP) or other relevant certification preferred.
