Summary The Pharmacist (Clinical Pharmacy Specialist) serves as an advanced practice provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. They support ambulatory teams, including PACT, anticoagulation, specialty medicine, and pain, for chronic disease management commonly seen in the ambulatory care setting.
