The Elizabeth Hospice is San Diego’s oldest non-profit hospice of  45 years . We are growing and looking for talented individuals to join our high performing team and support our mission. We serve all of San Diego county and south Riverside county with multiple hospice office locations.

The Hospital Liaison RN at The Elizabeth Hospice, initiates, establishes, develops and maintains professional relationships between hospice and various referral sources, e.g., hospitals, skilled nursing, assisted living facilities, RCFEs, etc., physician associations and independent medical clinics/offices in order to assure a consistent base and flow of patient referrals across the life span (perinatal, pediatrics, and adults).
