Job Summary:
Under the direct supervision of the Clinical Team Lead, the Patient Navigator–Community Health Worker plays a key role in supporting clients as they navigate behavioral health, physical health, and social service systems. The Patient Navigator will focus on patient and community engagement, outreach, care coordination, and advocacy.
