Summary
The Nurse Practitioner (sometimes referred to as an Advanced Practice Nurse) provides an enriched level of medical care with a goal of increasing wellness, prevention of illness and improving outcomes of care through early detection, wellness visits, and more timely diagnosis and/or treatment; Works with a multi-disciplinary team and individualizes treatment plans as residents move from practitioner(s) to provider(s) outside an EverTrue community, and upon returning home.
