Position Overview The Home Care Occupational Therapist provides skilled occupational therapy services directly in patients' homes. The role is responsible for evaluating functional abilities, identifying barriers to independence, and implementing treatment plans that support safe and effective performance of daily living activities in a home environment.
Core Responsibilities - Conduct in-home occupational therapy evaluations and establish patient-centered goals
- Develop and implement individualized treatment plans to improve functional independence
- Provide therapy interventions targeting activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
