Job Description:

Liquid Personnel is seeking a skilled and compassionate Occupational Therapist to join a dynamic Discharge to Assess (D2A) team for its client in Slough.

This is a fantastic opportunity to make a real impact by supporting timely and safe hospital discharges, ensuring patients receive the right care in the right place.

You will:
Support with Trusted Assessments as needed.

Use your clinical expertise to screen and process hospital discharges.

Facilitate smooth transitions from hospital to home or community settings.

Work collaboratively with multidisciplinary teams to deliver person-centred care.
Requirements:
HCPC-registered Occupational Therapist.
