**Responsibilities**

The **Care Manager Nurse** is responsible for leading and coordinating care for patients affected by or at risk for **Hepatitis C (HCV)** . This role involves providing patient education, care coordination, and collaboration with multidisciplinary teams to ensure successful screening, diagnosis, treatment initiation, and follow-up for individuals with Hepatitis C.
