Job Description
The Nurse Care Manager provides professional, patient-centered nursing care in an ambulatory setting, delivering both in-person and telephonic services to patients and families. This role supports a multidisciplinary plan of care by collecting and analyzing patient data, maintaining and updating care plans, and evaluating outcomes of nursing interventions. The Nurse Care Manager documents and communicates all care activities according to professional and organizational standards, delegates tasks to appropriate licensed and unlicensed team members, and ensures compliance with regulatory requirements, including Joint Commission and Patient-Centered Medical Home (PCMH) standards.
