Job Summary and ResponsibilitiesAs our Nurse Navigator at Good Samaritan Hospital, you will serve as the first point of contact for patients and their families/caregivers. You will work directly with them and their providers to eliminate barriers to timely care, facilitate flow through the system, and facilitate interactions with the clinical care team.
Educate patients on preoperative, intraoperative, and post-operative experiences. Facilitate communication between all members of the health care team.Coordinate office visits across comprehensive care teams for continuity of care delivery.Enhance each patient's experience by staying involved- visiting patients at appointments, assessing their needs, and relaying relevant information to providers.Collect data for best practice and quality of care.Job RequirementsGraduate of an accredited nursing programActive RN license in the state of Nebraska or co...
