MTM Pharmacist Redefine patient engagement through personalized, remote MTM services. Key Responsibilities: - Conduct telephonic or video MTM sessions for Medicare and commercial patients.
- Document clinical interventions and CMRs in compliance with CMS standards.
- Identify and resolve medication-related problems and adherence barriers.
Qualifications: - PharmD and MTM certification preferred.
- Prior MTM, chronic disease management, or ambulatory care experience.
- Ability to connect and build trust with patients virtually.
