Position Overview
JOB PURPOSE
To provide Primary Health Care services, Illness/Injury Prevention, and Chronic Disease Management using a population health approach in collaboration with the health care team.
To provide direct care to scheduled and unscheduled patients, involving all phases of the nursing process – assessing, planning, implementing and evaluating – with or without the physician or nurse practitioner on site.
QUALIFICATIONS
Active registration in NANB.Certification in Transportation of Dangerous Goods or willing to obtain within 6 months.Current BCLS certification.CNA Certification in Community Health Nursing or willing to complete within 2 years.Demonstrated knowledge and skills in providing direct patient care to people throughout the life span, in all stages of health and complexities of illness, from unpredictable situations and chronic illness to health promotion.Demonstrated knowledge and skills i...
