Key Duties
- To facilitate the delivery of a service which follows a person-centred ethos and promotes independence.
- To complete dynamic risk assessments and support plans.
- Promote wellbeing and appropriately manage risk.
- Ensure adherence to company and national policies and procedures.
- To work flexibly as required by the service; this may require some working outside of your normal hours to provide consistent support.
- You will be supporting people in accommodations or their homes, facilitating safe discharges and avoiding admissions.
- Collaborative working with a team of professionals with a wide range of backgrounds.
- Maintain compliance with mandatory & additional training in place to enhance your professional development.
