Provide counseling service to patients and caregivers
Provide psychosocial and financial assessment, coordinated case management and interventions to enable the patient/client to function optimally in the home environment and community at large
Assist patient to understand the diagnosis and implication on ADL functioning
Walk through with patients and caregivers and provide emotional support and motivation for them
Manage patient expectations and set realistic and achievable goals
Arrange for interim care or make referrals for long‑term step‑down facilities when necessary
Participate in ward rounds and case conferences to represent patients' social, psychological and economic circumstances
Responsible for volunteer management
Plan and oversee the volunteer programs, not limited to recruitment, screening, coordination, training and retention of volunteers
