MDS Coordinator

Manchester Center for Rehabilitation and Healing is looking for talented and motivated MDS Coordinator to join our dynamic team. The position is within a Skilled Nursing facility. Candidate should possess passion for geriatric population, must contain a license in the state of practice, be able to evaluate and treat independently without direct supervision, and posses the desire to perform functional therapeutic activities to assist residents improve in their experience.

MDS Coordinator Essential Job Functions


+ Oversees accurate and thorough completion of the Minimum Data Set (MDS), Care Area Assessments (CAAs) and Care Plans, in accordance with current federal and state regulations and guidelines that govern the process
