**Job Summary and Qualifications**

The Manager of Case Management supports the Director of Case Management in managing Case Management operations. This includes providing leadership for planning, management, implementation, integration, and coordination of Case Management in support of facility goals dedicated to excellence, effective leadership, and financial stability. The role supports the delivery of high-quality, cost-effective patient care through activities based on facility strategic goals and objectives.
