Description

This position provides quality nursing oversight, linkage of residents to primary care providers and/or mental health services, and monitors medication compliance of residents on a daily basis.

Responsibilities

+ Implements medication policies and procedures and ensures compliance with OASAS 820 Regulations.

+ Initiates and coordinates initial referrals with primary medical doctors and mental health providers and assists when necessary.

+ Assists with follow-up phone calls and advocacy on the resident’s behalf when necessary.

+ Maintains knowledge of the individualized care plan for residents and provides support to the residents according to their care plan.
