Job Description: As a home health nurse – LPN, responsibilities will be to assess patients' condition and initiate start of care face-to-face visits, perform evaluation tasks including vital signs and medication review, administer medication as prescribed by the patient's physician dress (or re-dress) wounds, provide patient education, coordinate with the interdisciplinary team regarding care plan progress, and make recommendations for devices/tools/medications that might improve patient's quality of life.
