The role of the Care Coordinator is to support the interdisciplinary medical team in provision of care through coordination of activities across and within healthcare delivery sites. The Care Coordinator will facilitate the interface between the medical home team and regional support teams in caring for patients with complex health needs. The assistance of the team will be pivotal in creating access, coordination, and interface between providers and patients as the needs of their population rises.
The Care Coordinator will provide direct and indirect assistance to the care manager RN, social worker, pharmacy, physicians and advanced practice providers.
