JOB SUMMARY
The LVN carries out nursing care plans for patients as per physician orders under the direction of a Registered Nurse. The LVN will perform daily activities with an attitude of humility. The LVN will approach each patient and coworker with respect and with the goal giving the highest quality patient care possible.
JOB DUTIES AND RESPONSIBILITIES
1. Under the direction of the RN, perform skilled nurse visit that assists in identifying the patient's physical, psycho social, and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations. This includes the prep time before the visit, visit documentation, and any follow up information that needs to be communicated to your supervisor. The documentation for this activity must be completed and in submitted with signature status within 24 hours of performing the visit. (EF)
