The Licensed Vocational Nurse (LVN) practices in the outpatient care setting under the direction and supervision of the Clinical Nurse Manager. Responsibilities Total Rewards of a Allied Health Professional The LVN is a member of the Patient Aligned Care Team (PACT) consisting of (Veteran, Provider, RN Care Manager, Clinical Associate and Clerical Associate) and Expanded Team (Family/Caregiver, Social Worker, Pharmacist, Dietician, Internal and Community-Based Services) involved in providing care to the patient and in developing the PACT plan.
