Licensed Practical Nurse

CVP is seeking licensed practical nurses to join our team in providing primary care services at the Dayton VA Medical Center in Dayton, OH.

Working hours are Monday through Friday, 8:00 AM to 4:30 PM EST.

Responsibilities

• Provide direct and indirect nursing care to veterans as part of the primary care patient aligned care team (PACT), contributing to patient assessment, determining care priorities, identifying nursing measures and therapeutic objectives, and evaluating patient outcomes under the direction of the VA nurse manager or designee.
• Obtain patient history and pertinent family history; assess patient condition and plan appropriate nursing care.
• Document plan of care and all care provided accurately and completely in the VA electronic health record (EHR/CPRS) prior to the end of each shift.
