With our 92 suite memory care and enhanced assisted living on Lakeville Campus we continue to bring the highest level of care, love and support to our residents. You are part of a multi nursing team, with 24/7 on site nursing coverage. We are currently seeking Evening (part time and full time) and Weekend Positions!

LPN Position Specifications:

• This position will ensure that the health and safety of all residents are met.
• Lead and direct your care team along with actively performing cares of each resident as needed.
• The LPN will collaborate with residents, families, physicians and Moments staff to create a person-centered plan of care that enhances the life and care of the resident.
