The Licensed Practical Nurse (LPN) will provide direct patient care services to geriatric patients at the VA Palo Alto Health Care System. Services will be delivered onsite in Community Living Centers and other VA facilities, meeting or exceeding national nursing care standards in a hospital and long-term care environment.

Key Responsibilities

• Provide routine and specialized nursing care to geriatric patients under RN and physician supervision


• Assist with patient assessments, vital signs, medication administration, wound care, and activities of daily living


• Maintain accurate and timely documentation in accordance with VA policies
