Position Overview

Licensed Practical Nurse (LPN)

Currently looking for EXPERIENCED Licensed Practical Nurse(s) (LPN) to fill multiple openings with a home care agency located in Raritan, NJ. Interested candidates should have a minimum one (1) year of documented experience required in a home health agency, primary care clinic or health facility. 

 Responsibilities of the Licensed Practical Nurse (LPN)
Provide thoughtful care to client & patients based on assessment and personalized Plan of Care

Monitor and track change in the client/patient’s condition and needs, keep the Director of Nursing and any other appropriate personnel informed

Proactively update Director of Nursing on health changes and concerns

Pay rate: $24 - $35hr., depending on assignment
 Requirements of the Licensed Practical Nurse (LPN)
Current unrestricted LPN license in New Jersey

Pediatric experience preferred
