**Job Summary and Responsibilities**

As our **Licensed Medical Social Worker** at Chandler Regional Medical Center, you will perform critical social work assessments and interventions for inpatient, outpatient, and Emergency Department patients. You'll lead patient-centered care by developing and implementing treatment plans, facilitating post-acute placements, and coordinating comprehensive resources for complex discharges and vulnerable populations. This role is central to crisis intervention, patient advocacy, and ensuring developmentally appropriate care across all served populations, from mentally ill to chronically ill.

Every day you will manage psychosocial care, including crisis intervention, counseling, and referrals for diverse issues like abuse, family challenges, and grief.
