Summary Delivers fundamental knowledge-based care to assigned clients while developing technical skills. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self management, and customer satisfaction throughout the continuum of care. Follows procedures per established polices and guidelines, Responsibilities The functions and duties of the Interventional and Specialty Care Coordinator include but are not limited to the following: Administer medications and treatments according to policy. Monitor patient responses and promptly report changes. Recognize urgent or emergent situations and perform appropriate interventions. Manage phone calls, voicemail, secure messages (SM), My HealtheVet (MHV) communications, and alerts during clinic hours to ensure timely resolution.
