The Advanced Practice Provider (Nurse Practitioner/Physician Assistant) within the ambulatory setting works in collaboration with, and is an integral member of, a multidisciplinary health care team. The PRIMARY CARE APP provides comprehensive care in a Internal Medicine practice.

Overview:

• Provides primary care to patients across the life span (family medicine) or adult patients (internal medicine).

Assessment/Diagnosis and Management of Patients in Primary Care Setting

• Provides preventive health, including well visits, screenings, and immunizations; as well as acute and complex care visits requiring diagnosis, treatment/management, and referral to specialists or higher level of care when necessary.
