Schedule: Monday Friday Full-Time

Practice Setting:

• Comprehensive outpatient PACE program for seniors with chronic health needs
• Interdisciplinary team environment
• Community-based care to maintain patient independence
• Services include primary care, therapies, nursing, home support, and more

Role Overview:

• Medical oversight of participant care
• Clinical supervision of PCPs and APPs
• Leadership of QAPI and quality measures
• Participation in intake assessments and care planning
• Ongoing evaluation and management of chronic conditions
• Guidance to interdisciplinary care team
• Medical policy development
• Limited on-call (PACE model; discussed at interview stage)
