Position Overview

Primary Care Physician (Hybrid, On-site & Remote) - Colorado Springs, CO
Schedule: Full-Time Monday-Friday Hybrid On-site & Remote
Pay Range: $220k - $250k Annual
Overview: We're seeking a compassionate Primary Care Physician to join our SNF Transitions Team, delivering subacute and palliative care to geriatric patients. Ideal for physicians with hospitalist or SNF experience seeking a meaningful role with excellent work-life balance.
Responsibilities:
Provide subacute/palliative care in SNF
Lead care planning and IDT meetings
Guide goals-of-care and advance care planning
Requirements:
BE/BC Physician, CO license; active DEA
2+ years hospitalist/SNF/post-acute experience is required
Strong in geriatrics, chronic disease, and palliative care
Bilingual is a plus
