Summary:
The House Clinical Coordinator (HCC) is a registered nurse who is accountable for effective coordination of patient care needs. The HCC performs a variety of clinical and managerial duties within a hospital as assigned. The HCC coordinates care for patients, acting as a liaison between departments as well as between patients and their families and health care professionals. The HCC will assist with monitoring of patient care delivery and resolve any issues that arise related to patient safety, clinical outcomes, or patient experience as they are notified by the physician or staff. The HCC is an active member of the multidisciplinary team and collaborates in the development, execution and evaluation of the multidisciplinary plan of care. He/She considers factors related to ethics, safety, effectiveness and cost in planning and delivering care.
