Location: Northern Wisconsin (Green Bay, Oshkosh, Manitowoc, Sheboygan & Grafton Region)

Setting: Hospital-Based Hospice Liaison | Travel Assignment

Position Overview

We are seeking an experienced Hospice Liaison Registered Nurse to coordinate hospice referrals and guide patients and families through end-of-life care transitions. This role involves collaborating with hospital teams, assessing patients for hospice eligibility, educating providers and families, and facilitating admissions into hospice services.

Assignments will typically cover 1–2 hospitals at a time , with travel throughout the northern Wisconsin region as needed.
