In this role, you will partner with hospital teams to support patients and families through care transitions, ensuring timely access to appropriate services including hospice, palliative care, and home health.

The Hospice Liaison – Clinical plays a critical role in clinical assessment, care coordination, and relationship management, while supporting referral growth through strong partnerships with case managers, discharge planners, and social workers.
