Role and Responsibilities


Provide personal care and supportive services to individuals in the community , in the privacy of their home.





Essential Functions


+ Provide health - related services as outlined in the plan of care , authorized by a physician , and established by a registered nurse or licensed therapist , under the direction of the Aide Matrix established by the NYS Department of Health.

+ Provide assistance with personal hygiene, housekeeping and/or other related supportive tasks to a patient with health care needs in his/her home.

+ Immediately report changes in client’s condition to supervising professional.

+ Ensure the safety of the client , clie nt’s family and/or the employee.

+ Maintain HIP A A.
