**Main Responsibilities:**

+ You will conduct comprehensive in-home evaluations and examinations
+ You will use both in-person and telemedicine capabilities to deliver care
+ You will develop and implement personalized treatment plans
+ You will manage acute clinical issues in the home
+ You will maintain accurate documentation
+ You will participate in medical team meetings
