The Health and Wellbeing Coach provides personalised support to individuals to empower them take control of their own health and wellbeing with a focus on lifestyle behaviour change, inclusive but not limited to the core areas of: * Healthy Eating and Nutrition * Smoking Cessation * Physical Activity * Weight Management * Alcohol consumption * NHS Health Checks (outreach) As a Health and Wellbeing Coach, you will facilitate people in establishing and attaining health-promoting goals, set by the person to change lifestyle-related behaviours, with the intent of reducing health risks, improving self-management of chronic conditions, and improving quality of life. The Health and Wellbeing Coach is also responsible for delivering face-to-face, telephonic, and digital health coaching support to individuals from a wide range of community and healthcare settings via a combination of one-to-one and group-based support in line with need, location and offer.
