Geriatric Support Services Coordinator $28.58/hour
Hiring Bonus: GSSC candidates who are hired and begin employment on or between 4/1/26 and 6/30/26 are eligible to receive a hiring bonus in the amount of
$2,000. This bonus will be paid in one lump sum on the next regularly scheduled pay date after you successfully complete six months of employment. This bonus is taxable, and all regular payroll taxes will be withheld.
Position Summary : This position is responsible for initial member assessments, reassessments, and ongoing case management of Senior Care Options (SCO) and One Care members in compliance with regulatory and agency guidelines.
