The Clinical Pharmacist is an integral member of the DAFHT interdisciplinary team, supporting the organization’s vision and strategic objectives through collaborative primary care and improved medication management.

DUTIES AND RESPONSIBILITIES

• Collaborate with physicians and the interdisciplinary team to promote optimal, patient centered care
• Conduct comprehensive medication assessments to identify, prevent and resolve medication therapy problems
• Develop individualized care plans and monitor therapeutic outcomes
• Provide accurate, timely, and evidence-based drug information to physicians and the interdisciplinary team to support safe and effective prescribing decisions
• Conduct Medication Reconciliation at transitions of care to ensure medication regimens are accurate and safe
