In this role, you will work collaboratively with multidisciplinary colleagues to deliver person‑centred care for individuals living with frailty. You'll assess functional abilities, develop tailored interventions, and empower patients to achieve their goals in the community.
As part of Cornwall's integrated care approach, you'll help shape seamless working across health, social care and voluntary sectors, ensuring the local population receives joined‑up support at place.
