In this role, the successful candidate provides safe, quality, therapeutic, and age-appropriate nursing care using evidence-based practice. Uses clinical expertise in assessment, clinical decision making, clinical practice and patient education. The Field Nurse coordinates the care provided with the interdisciplinary team to promote optimum patient outcomes and satisfaction.

Job Responsibilities:

Completes a comprehensive nursing assessment including but not limited to physical, functional, psychosocial, emotional, cognitive, sexual, cultural, age related, environmental, spiritual, transpersonal, and economic factors in a systematic and ongoing process while honoring the uniqueness of the patient

Derives the diagnoses and needs from the assessment data, validates the diagnoses, and collaborates with the patient, family, caregivers, and physician to develop an individualized plan care to attain expected quality outcomes
