Family Stabilization Case Manager
8:00 AM to 5:00 PM, Monday through Friday
Implementation of Family Stabilization (emergency assistance) and case management programs.
Occasional assist with Special Distribution projects.
Complete initial client interview, required paperwork, and intake in agency data tracking system for emergency assistance.
Assess, and assist families in the development of a self-sufficiency plan and track client progress in agency database.
Work in conjunction with Family Stabilization Committee in determining client eligibility for assistance requests.
Maintain confidentiality of all client records, information, and communications in accordance with CAPWN policy and HIPAA privacy and security requirements, while providing a supportive, trauma‑informed environment for every client interaction.
