If successful this role will involve close collaboration and direct engagement with patient’s local care providers, services and sectors to provide and promote a holistic and personalised approach to patient support.

This will include navigation and co-ordination of surgical and supportive care, with a requirement to signpost patients towards specialist resources to enable them to maximise the quality of care and support they receive.
