ED Social Worker - Weekend (Permanent)



Hours: 12hrs, Fri-Sun, 0800-2030.



Requirements:

• MD LMSW or LCSW-C


• 1+ years of discharge planning experience in an inpatient setting


• ED experience is a plus 


• EPIC experience a plus 


• Training for the weekend shift will be during the week

Fully Benefited position!  



Responsibilities: Early Identification, high risk screening, and readmission screening of all patients. Services include: adjustments to illness counseling, safety assessments, discharge planning, identification of obstacles and barriers to discharge through proactive screening and assessments and coordination with the multi-disciplinary team.
